LANCASTER, Ohio (AP) -- The federal Occupational Safety and Health Administration says it has cited a central Ohio glassware manufacturer for 12 alleged safety violations.

OSHA said Thursday that it began investigating Anchor Hocking LLC in Lancaster after a worker's finger was amputated while he performed machinery maintenance March 18. The government says workers had not been trained in some procedures to prevent machinery from operating accidentally.

The government also says the company failed to protect employees from falls off an open-sided work platform and due to floor holes. OSHA says other violations included failure to adequately record work-related injuries

Anchor Hocking faces proposed penalties of $113,800.

The company has 15 days to comply, request a conference or contest the findings. A telephone message seeking comment was left with the manufacturer on Thursday.
